Make a verbal request or file a written request for an original copy or reissue of diploma.  Provide data as to last Section and graduation year, and proof to identity and vital corrections. Receives and notes down request and verifies identity and relation of client to the graduate, valid proofs, senior section and year of graduation. 2 minutes Guidance Office Staff, Principal
Proceed to the Cashier for payment of request. Issues Official Receipt and receives payment. 2 minutes School Cashier
Wait or return to claim the requested Diploma. Verifies data, search for or print, logs, then issues Diploma. 1 minutes Guidance Office Staff

 

Form 137

As the Client, you
The Responsible Office
It will take
Person(s) Responsible
Make a verbal request or file a written request for a certified copy of original Form 137. Provide data as to last Section and year attended, and proof to identity and vital corrections. Receives and notes down request and verifies identity and relation of client to the student, valid proofs, year and last section before exit. 2 minutes Guidance Office Staff, Principal
Proceed to the Cashier for payment of request Issues Official Receipt and receives payment 2 minutes School Cashier
Wait or return to claim the requested Form 137 Verifies data, searches for record, encodes or prints from data base, gets finished document signed by the Principal, logs, then issues Form 137 5 minute Guidance Office Staff, Principal

 

Certifications

As the Client, you

Person(s) Responsible
Make a verbal request or file a written request for a Certification of Enrolment, Graduation, Good Moral Character, Honorable Dismissal, Medium of Instruction, Curriculum Offered, CAV, etc.  to the Guidance Office or any other office concerned.  Provide data as to last Year and Section attended, and proof to identity. Receives and notes down request and verifies identity and relation of client to the student concerned, valid proofs, last year and section attended 2 minutes Guidance Office Staff, Principal
Wait or return to claim the requested Diploma Verifies data, search for record print, logs, then issues Certification 5 minutes Guidance Office Staff
